Ending 1 is the basic Mom kill, while Ending 20 requires completing the full Repentance alt path ascent and defeating The Beast. Core Endings (1-12)

Endgame Endings (16-20)

Ending 16 unlocks by defeating Mega Satan — the two-phase fight behind the golden door requiring both Cathedral and Sheol key pieces. Ending 17 follows a similar Mega Satan path with different cutscene content. Ending 18 unlocks by defeating Mother on the alt path Corpse II floor. Ending 19 triggers after beating Dogma during the alt path ascent sequence — Dogma is the penultimate boss before Home. Ending 20 is the true final ending, unlocked by defeating The Beast on Home during the alt path ascent. Ending 20 completes the Repentance narrative arc and contributes to full game completion.

How to Reach Each Ending Path

• Ending 1-11 — Beat Mom's Heart or It Lives! and walk into the light. Repeat wins unlock subsequent endings.
• Ending 12 — Beat It Lives!, return to the starting room, bomb the wall behind you, and enter the strange door with The Fool card from the boss room.
• Ending 14 (Cathedral) — Take angel room deals and avoid devil deals. Reach Cathedral through the angel path.
• Ending 15 (Sheol) — Take devil deals. Reach Sheol through the devil path.
• Ending 16-17 (Mega Satan) — Collect both key pieces from Uriel and Gabriel angel mini-bosses. Use keys on the golden chest in Dark Room/Chest starting room.
• Ending 18-20 (Alt path) — Enter alt path on Depths II, progress through Downpour/Mines/Mausoleum/Corpse, beat Mother, complete ascent, defeat The Beast on Home.

Ending Unlocks and Progression Impact

Each ending permanently unlocks content on your save file. Ending 1 unlocks Eden and The Womb. Ending 12 unlocks the Blue Womb and Hush boss access. Mega Satan endings unlock Mega Satan fight availability and related items. Repentance endings 18-20 unlock alt path content permanently — after Ending 18, Mother fights become available on all future alt path runs. The Beast ending completes the Repentance story and is required for the Beast completion mark on every character toward Dead God.

Ending Speedruns and Routing

Speedrunners route endings by minimizing unnecessary floors and boss fights. The fastest ending (Ending 1) requires only a Mom kill — achievable in under 5 minutes with Azazel. Ending 12 routing requires reaching It Lives! within 20 minutes for Boss Rush access plus the strange door sequence.
