The Binding of Isaac: Rebirth Birthright Guide

Birthright is a special passive item introduced in Repentance that grants a unique, character-specific power-up when collected. Unlike other items, Birthright's effect changes depending on which character picks it up — Isaac can hold two active items simultaneously, Magdalene gains an extra heart container and stronger Yum Heart, and The Lost receives extra lives that respawn him in the same room. Birthright appears exclusively in a dedicated Birthright item room that generates on alt path floors after you defeat Mother for the first time on a save file.

How to Find Birthright

Birthright rooms appear on alternate path floors — Downpour, Mines, Ashpit, Mausoleum, and Gehenna — after Mother has been defeated at least once. The room contains a single Birthright pedestal with no other items. You can only take Birthright once per run, and its effect persists for the remainder of that run. To consistently access Birthright, you must navigate the alternate path by bombing the unique wall in Depths II (or the equivalent alt route entrance on other floors). After beating Mother, subsequent alt path runs will include Birthright rooms.

Strongest Birthright Effects

Birthright on Tainted Characters

Every Tainted character has a unique Birthright effect that enhances their inverted mechanic. Tainted Isaac's fourth item slot is the most impactful — it adds an entire extra passive item to his three-slot inventory, effectively a 33% power increase. Tainted Lost gaining a second per-room Holy Mantle hit transforms him from a precision character into a more forgiving one. Tainted Cain's Birthright causes items to drop additional pickups when split, amplifying his economy engine. Tainted Magdalene's Birthright reduces her health drain rate, making her more sustainable across long runs.

Birthright Strategy Tips

• Prioritize alt path when Birthright matters — Characters like Isaac, Tainted Isaac, and The Lost gain enormous power from Birthright. Path alt on key Dead God runs.
• Birthright is run-only — Unlike unlocks, Birthright does not persist between runs. You must find it each time on the alt path.
• One per run — You cannot stack Birthright or re-enter Birthright rooms for a second copy.
• Works with other items — Birthright is a passive item that synergizes normally. Isaac's dual active slots work with Schoolbag and Battery combos.
• Not in Greed mode — Birthright rooms do not appear in Greed or Greedier mode since those modes lack alt path floors.

For characters where Birthright is less impactful — like Tainted Azazel whose Brimstone is already strong — you may skip the alt path detour in favor of a faster Cathedral/Sheol clear. Evaluate whether the Birthright bonus justifies the harder alt path floors with their unique mini-bosses and environmental hazards. When planning completion mark runs that require Mother and Beast marks anyway, always grab Birthright since you are already on the alt path.
